Dr Ben Davis Appointments Terms & Conditions

Dr Ben Davis provides both face-to-face and remote (virtual) consultations in sexual medicine. These terms and conditions form a binding contract (‘Contract’) between you, the patient (‘Patient, ‘You’ or ‘Your’), with Dr Ben Davis (‘I, ‘Me’, or ‘My’). Your use of the Services are bound by these Terms and Conditions so please take the time to read them carefully and make sure you have understood them.

Appointments:

  • Initial appointments are face-to-face at 9 Harley Street. This is to ensure any physical examinations can be done and so I can get to know you well.

  • Follow-up appointments can be a mix of online or face-to-face at 9 Harley Street, depending on your preference.

  • Appointments are subject to availability. I will endeavour to honour a request to be seen on a particular week but this cannot always be guaranteed.

  • Initial appointments are 50 minutes. Follow up appointments can be 30 or 50 minutes, depending on clinical need.

 Payment:

  • Payment is due in order to confirm any appointment booking. The appointment is not confirmed if payment has not been made.

  • All appointments are to be paid for by self-payment prior to any appointment.

  • I’m afraid I do not deal directly with insurance companies. You can contact your insurance company about reimbursement, although this tends to be variable depending on your insurance.

  • If applicable payment for testosterone care membership can be made in full at the beginning of treatment, or in instalments.

  • Fees may change over time. I will endeavour to keep you updated in any changes to fees. Current fees are always available on my website.

 Appointment Cancellation Policy:

  • The full fee remains payable if an appointment is cancelled for any reason within 24 hours of the appointment.

  • 50% of the fee will be payable if the session is cancelled between 24 and 48 hours of the appointment.

Consent

  • I will explain to you the nature and purpose of any treatment I propose, the benefits and side effects that you might experience. I may ask you to sign a consent form for treatment prior to prescribing any medical treatment.

Confidentiality:

  • Confidentiality is incredibly important and I take confidentiality and data protection extremely seriously.

  • Occasionally I may need to share information about you with other people, such as when making a referral or in writing to your GP. I will always ask for your consent prior to doing so.

  • Exceptionally, where there is a risk of harm to you or others or when required by law, I may need to make limited disclosure of the content of our work to a third party. Wherever possible I will inform you of this in advance.

Men’s Testosterone Care Membership:

  • Membership for ongoing testosterone care consists of a package of appointments, required investigations, prescriptions and support. Having regular follow up and monitoring is incredibly important for the safety and success of testosterone therapy.

  • First year of membership consists of a package of four appointments, four blood tests including lab and phlebotomy fees and prescriptions as needed. It does not include extra appointments, investigations or blood tests, neither does it include the cost of any medication, which you will need to pay a pharmacy separately for. The cost for year one is £1500, or twelve payments of £150.

  • The second year of membership and beyond consists of two appointments and two blood tests, including phlebotomy fees, and prescriptions. Again it does not include the cost of any medication, extra appointments, extra investigations or extra blood tests. The cost for year two and beyond is £750 or twelve payments of £75.

  • Membership is there to help support you with anticipating the costs of treatment, as well as emphasising the importance of regular blood testing and follow up.

  • If you fail to attend follow up appointments or have regular blood tests, I will not be able to continue prescribing for you.

Cancellation of membership

  • Membership can be cancelled at any point after the first six months of care. You will be eligible for a refund of any unused part of the membership, depending on how many blood tests and appointments you have had already had during your membership year.

  • Membership may be ended by me at any point if you fail to attend follow up, for blood tests, if your behaviour is abusive, I believe you are abusing prescribed medication or changing your dose without discussing with me first or in the unlikely event there is a break down in our therapeutic relationship. I will give you 30 days’ notice in the unlikely event of the need to terminate the membership early.

Prescriptions

  • Please note there is a difference between prescriptions and medication costs. A prescription is a formal communication between a doctor and a pharmacy authorising them to dispense a specific drug for a specific patient. I do not charge you extra for prescriptions completed during an appointment or as part of membership.

  • I will provide you with a prescription and often send it to a recommended pharmacy with your agreement. The pharmacy will then charge you for any medication and deliver it to you. This means I do not charge a mark-up on any medication and you can find the cheapest supplier if you wish.

  • I am not responsible for the fulfilling of prescriptions; this is down to individual pharmacies. There are often shortages of prescription medicines in the UK, which can be frustrating. I will in no way be responsible for an interruption of any treatment due to such stock shortages. I will support you in any way I can if this happens, including recommending alternative medication.

  • The cost of medication delivered during a consultation such as vaccinations will be added to your bill and will not be included in the consultation cost. 

Face to face contact outside sessions

  • Your confidentiality is of utmost importance to me. Should we meet outside of your sessions, please be aware that I will not usually directly greet you. This is to protect both your privacy and to maintain confidentiality. If you acknowledge me openly, then I will respond to you in an appropriate professional manner.

Complaints

  • If you have a complaint about me or the service, I would urge you in the first instance to raise any issues with me directly. I will respond to any concerns openly and immediately. If you are dissatisfied with the response you can contact Phoenix Hospital Group directly or the General Medical Council.

Suspending the Service, Unforeseen Events, & Events Outside My Control

  • I am not responsible for events that are out of my control or unforeseen circumstances. If the clinic space is closed or the running of the services is affected by an event outside my control or an unforeseen circumstance then me or my secretary will contact you as soon as possible if you have an appointment booked to let you know and we will take steps to minimise the effect of the event or circumstance. Provided I do this, I will not be liable for short-term closure or disruption caused by the event or circumstance in question

  • I may have to suspend my services for any reason, including but not limited to: changes to regulatory requirements imposed by the General Medical Council or the Care Quality Commission. Where possible, I will contact you in advance if you have an appointment booked to tell you I will be suspending the services and try to find you an alternative practitioner. If I have to suspend the services for longer than fourteen (14) days you will be entitled to a full refund for any appointments already paid for that you have been unable to attend due to the suspension.

  • I reserve the right to terminate care for any patient who is not abiding by this agreement.
